It is logical to intercept a malocclusion as early as possible and to reduce or in rare instances, avoid multi banded mechano therapy at the sensitive teenage period. Why allow an unfavorable skeletal, dental and soft tissue relationship to exist for a number of years if it can be corrected or practically corrected, early, with a minimum of appliance treatment time? The term serial extraction was first introduced by Kjellgren in 1929. Teeth may be extracted with greatest of ease during a so called serial extraction but special knowledge is required to carry out this procedure successfully. Serial extraction is not easy, as so many dentists mistakenly believe.

Dental Crowding

Dental crowding is the most common chief complain of a prospective orthodontic patient. Orthodontic treatment of crowding through proper alignment of the teeth may become difficult in the presence of tooth size discrepancies. In order to obtain ideal occlusion, it is necessary to know if and where a significant tooth size discrepancy exists.

Discrepancy in tooth size is defined as a disproportion among the sizes of the maxillary and mandibular teeth. The tooth size discrepancy should be compensated for in the treatment planning by considering esthetic bonding, prosthetic recontouring, stripping of enamel, extraction, leaving spaces or changing the desired anterior overjet and overbite.

Periodontitis

Periodontitis is the result of a sequence of inflammatory reactions caused by the dental biofilm. Attachment and bone loss result from a complex interaction from virulent pathogen in its local environment and the suceptible host. The disease process can be arrested but the damages are often irreversible.

Change in gingivo-mucosal morphology and bone topography as a result of disease is unavoidable. To prevent the disease progression the site’s cleanliness should be maintained. In restoring the tooth’s function and esthetics, attention should be given to ensure that no mechanical reccesses are present. There should be no violation of biologic width. Some of the outcomes of periodontal disease and periodontal therapy are increased triangular spaces, open embrasures, secondary occlusal trauma and involvement of furcation.

Ideal Occlusion

Ideal occlusion is an occlusion compatible with the stomatognathic system, providing efficient mastication and good esthetics without creating physiologic abnormalities.

Semiadjustable articulator reproduce an individual's linear mandibular movement.

Balanced Occlusion is an occlusion wherein all teeth contact in maximum intercuspation and during eccentric mandibular movements. Fully balanced occlusion is ideal for restoration with complete dentures. It can also be used for osseointegrated implant supported overdentures. A theory on movement for balanced occlusion states that forces are generated horizontally instead of vertically.

Balanced occlusion is based on three classic theories:

  • Bonwill’s three points of occlusal balance
  • Spee’s curve of spee
  • Monson’s sperical theory

A balanced occlusion in natural dentition with normal periodontium is hard to find. It is usually the result of advanced attrition. Balanced occlusion is appropriate for complete denture treatment.
